Family Ark provides case management services to youth and families in southern Indiana. The Community-Based Case Manager provides case management services in the home and collaborates with team members, schools, families and other professionals to meet each family's specific goals and needs. This individual works with DCS-involved families who have had a substantiated incident of abuse and/or neglect. This position reports to the Community-Based Program Manager.

Key Responsibilities Include
- Facilitates case management services primarily in the home of the client.
- Maintains all client case records by documenting client contacts, as well as contacts with the Department of Children's Services, schools, parents, and other professionals.
- Develops treatment plans, revises treatment plans and writes and submits monthly reports to the Department of Children's Services.
- Participates in case conferences and court proceedings as necessary for client care.
Minimum Requirements
- Bachelor's degree in social work, psychology, sociology, or a directly related human service field.
- One year work or internship experience in the social service field.
- Strong verbal and written communication skills.
- Ability to interact effectively with people from diverse social and economic backgrounds
